In consideration of my child's (name listed above) opportunity to participate in Georgetown Bible Church's activities and programs, I acknowledge and accept the risks of injury associated with participation and transportation to and from any and all activities and programs of Georgetown Bible Church. I accept personal financial responsibility for any injury or other loss sustained during the activities or programs of Georgetown Bible Church or during transportation to and from such activities and programs, as well as for medical treatment rendered to my child that is authorized by Georgetown Bible Church, its leaders, employees, volunteers, or agents. I specifically consent to allowing my child to be transported to receive emergency care and to be responsible for all financial charges for such emergency care.
I release and promise to indemnify, defend and hold harmless Georgetown Bible Church, its leaders, employees, volunteers, and agents from any and all injury or loss resulting directly or indirectly from the activities and programs of Georgetown Bible Church or transportation to and from such activities or programs, whether such injury resulted from the negligence of Georgetown Bible Church, my child, or otherwise.
I, as the parent/guardian of (child listed above), hereby grant Georgetown Bible Church, its representatives, and employee's permission to photograph and use images of my child during activities associated with Jungle Journey VBS 2025 at Georgetown Bible Church. I understand that these images may be used for promotional purposes, including but not limited to:
-Display on the Georgetown Bible Church's website and social media platforms.
-Use in print materials, such as brochures, newsletters, and flyers.
-Display for future VBS events or church events.
I understand that:
-No compensation will be provided for the use of these images.
-Georgetown Bible Church will not publish my child's name or any other personal information with any of its publications, unless otherwise agreed upon.
-I have the right to revoke this permission at any time by notifying Georgetown Bible Church in writing.
